Takuo HASHIMOTO, M.D.,Toshinori KANKI,M.D.,Satoshi ABE, M.D. Norio NAKAMURA, M.D. Department. of Neurosurgery, The Jikei University School of Medicine (Received 4 September 1989, Accepted 10 January 1990) Abstract Flow patterns of the velocity measured by transcranial Doppler(TCD) were compared with the changes of intracranial pressure(ICP) in patient with subarachnoid hemorrhage and severe head trauma in our department. The flow velocities in the middle cerebral artery or internal carotid atrery were measured using TCD in 12 patients who developed brain death due to progressive increased ICP. Our studies demonstrated that flow patterns were influenced in response to increased ICP. The flow pattern showed a low resistance high flow profile in physiological state. On the other hand, TCD demonstrated a high resistance low flow in reply to increase in ICP.In the stage of marked increase in ICP, TCD revealed progressive reduction of mean flow velocity and diastric negative flow. Finally an oscillating flow pattern or spiky systolic flow pattern was estimated by TCD in brain death patients. TCD is beneficial to evaluate the arrest of intracranial circulation in brain death.

Diagnosis of brain death by transcranial Doppler sonography.

Delays in diagnosis of brain death have definite consequences for the organ retrieval and transplantation process. It is advisable to use accurate diagnostic methods. Transcranial Doppler sonography is a well-accepted technique for diagnosing cerebral circulatory arrest. However, in some cases, the access to the intracranial circulation via the temporal window is difficult. In 43 brain-dead pat...

Transcranial doppler sonography diagnostic value for the cerebral flow velocity changes in the interictal phase of classic migraine

Background: An imbalance of the cerebrovascular response during functional activation of the brain has been postulated as a factor in the pathophysiology of migraine. The purpose of this study was to determine the transcranial doppler sonography (TDS) diagnostic value for the cerebral flow velocity changes in the interictal phase of classic migraine. Methods: This study was carried out on 46 pa...

Diagnosis of brain death by transcranial Doppler sonography

The blood flow velocities in the basal cerebral arteries can be recorded at any age by transcranial Doppler sonography. We examined nine children with either initial or developing clinical signs of brain death. Soon after successful resuscitation increased diastolic flow velocities indicated a probable decrease in cerebrovascular resistance; this was of no particular prognostic importance. As s...

Coregistration of transcranial Doppler - sonography and EEG in the diagnostic procedures of detection of brain death

Brain death (BD) diagnosis is the clinical assessment of the irreversible loss of function of the entire brain, including the brainstem. Transcranial Doppler (TCD) examination is used in several countries as a confirmatory test to assess cerebral circulatory arrest (CCA), but is not feasible in patients with absent temporal bone windows. TCD is highly specific, but less sensitive because of fal...

Cerebral hemodynamic in patients with major β-Thalassemia using transcranial Doppler sonography

Background: β-thalassemia, a severe form of anemia, is an inherited blood disorder characterized by growth retardation, splenomegaly, and bone abnormalities. Complications related to treatment-induced iron overload also affect the quality of life of patients with major β-thalassemia. Some recent studies indicated cerebral hemodynamic disorders and increased risk of stroke in these patients. The...
